Follow up on developmental and family screenings to ensure timely referrals and service access. Support families in navigating health, housing, food, transportation, and behavioral health systems. Provide warm handoffs to community-based services and maintain a manageable caseload for in-depth support. Track family progress and maintain accurate documentation in alignment with program protocols. Family Engagement & Support

Build trusting relationships with families to support long-term engagement and stability. Participate in family engagement activities such as Parent Cafs and community events. Collaborate with Early Childhood Specialists and Program Administrators to ensure coordinated support. Community Collaboration
