Under the supervision of the director, the coordinator provides collaboration and leadership within the palliative care programs and assists in strategic planning for program development and growth. Supports the palliative care programs for each entity, identifying and supporting opportunities to improve patient and family experiences, improve efficiency and effectiveness of resource use, and ensures compliance with legal, regulatory, and clinical policies and procedures. Analyzes and articulates outcomes demonstrated by the palliative care programs through regular report writing and dissemination of information. The value you'll bring to the team: Complete comprehensive consultations and follow-up care to include physical, psychosocial and spiritual assessment. Provide recommendations for interventions related to palliative care principles that will optimize patient care plan. Evaluate patient and family responses and intervene in a timely manner to ensure optimal patient outcomes.
