The Multidisciplinary Clinic Oncology Nurse Navigator (ONN) plays a critical role in facilitating timely, coordinated, patient-centered cancer care. Working within a multidisciplinary oncology clinic (MDC), the ONN ensures patients are prepared for their MDC visit, understand their diagnosis and treatment options, and experience a smooth, streamlined evaluation involving multiple oncology specialists. After the MDC appointment and care plan discussion, the ONN transitions the patient to their assigned long-term oncology nurse navigator, ensuring continuity of care and a seamless handoff. Essential Job Functions: Triage new patient referrals to the MDC; ensure workup (imaging, pathology, labs) is complete prior to the visit. Coordinate multidisciplinary consultations (e.g., surgical, medical, and radiation oncology) during MDC sessions. Communicate proactively with providers to prepare case summaries and ensure all team members are aligned. Serve as the primary point of contact for patients and families before, during, and immediately after the MDC visit. Educate patients on their diagnosis, treatment options, and care pathways as discussed during the MDC visit. Ensure a clear and timely handoff to the patient's designated long-term oncology nurse navigator, including a summary of the MDC plan, next steps, and any outstanding care needs. Collaborate closely with the permanent navigator to support smooth care transitions and ensure no delays in follow-up or treatment initiation. Collaborate with MDC physicians and staff to ensure seamless transitions across treatment modalities. Participate in tumor board discussions and document decisions and care plans. Facilitate follow-up appointments, additional testing, or referrals as needed. Maintain accurate EMR documentation using standard MDC navigation templates. Track patient metrics, navigation outcomes, and delays in care. Contribute to ongoing MDC quality improvement efforts. Required Qualifications: Registered Nurse with a current Illinois license Graduate of a National League for Nursing accredited School of Nursing with BSN Minimum of two years hematology/oncology clinical nursing experience Preferred Qualifications: Masters of Science in Nursing OCN Certification ONS Chemotherapy Provider Experience in a nurse navigator role or working within a multidisciplinary oncology clinic Position Details: Job Type/FTE: Full Time (1.0 FTE) Shift: Days
