Case Manager

Position Purpose: The Case Manager will advocate for assigned clients and provide targeted case management and outreach services to individuals with mental health and/or substance use disorders. They will ensure clients are connected to appropriate care, support and vocational programs to enhance community functioning and achieve rehabilitation, resilience, and recovery goals. The Case Manager will collaborate with a multidisciplinary team to deliver case management and behavioral health services to clients and their families, integrating recovery principles into clients' daily lives. Job Requirements: Bachelor's Degree in Psychology, Social Work, or other related field. Valid driver's license. Qualifications: Demonstrated education, training or experience. Adequate written skills to accurately complete required documentation within the time frames prescribed. Ability to work independently. Proficient in Microsoft Office and Windows environment. Ability to present and coordinate lectures for clients, parents, staff or community resources. Desire to learn and grow as a professional. Skilled in behavior management techniques. CPR certification adult, child and infant within 90 days of hire. Ability to meet company's auto insurance requirements. Complete Iowa HHS Mandatory Reporter Training for Dependent Adult and Child within six months of hire. Essential Responsibilities: Undertake primary case management and care coordination responsibilities for designated clients. Provide crisis intervention services and medication monitoring as necessary. Adapt verbal and written communication to the language and style of the client and their family. Work hours deemed necessary for the responsibilities assigned. Ensure that client service records are maintained consistent with rules and regulations. Maintain communications with all staff (e.g. admissions, support service updates and discharge plans). Coordinate services with referral sources and other community resources. Familiar with court proceedings and available to make court appearances on behalf of Rosecrance clients. Present workshops, lectures and training to clients, families, employees and community groups. Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law. Deliver exceptional customer service consistently to every customer. Serve as a role model for other staff, clients, and customers and demonstrate positive guest relations in representing Rosecrance. Assume other related responsibilities as delegated and assigned.
