Family Finding Coordinator

Position Location: Schenectady-Park Place - Schenectady, NY Position Type: Full Time Salary Range: $25.00 - $25.00 Hourly Travel Percentage: Up to 25% Position Summary: This position will lead the department in finding families and engaging supports for children and youth in foster care without permanency. This position is the connector between counties, the youth's multi-disciplinary teams, and identified natural supports to engage and coordinate possible placement. Essential Duties and Responsibilities: Lead the service of family finding within the division, this includes speaking to counties, and internal and external stakeholders involved in identifying and securing families for visitation and placement. Collecting information about youth and family by interviewing youth, family, family connections, and community supports. Identify and document at least 25 family/kin for each child through diligent search activities including the internet, social media and databases, case record, and interviews. Facilitate communication with the treatment team and provide referring agencies with accurate family documentation, including detailed genograms, family contact information, and identified family supports. Connect with potential families and serve as a connector between the program, DSS, family, and youth for potential placement and resources. Other duties may be assigned. Education and/or Experience: Requires a Bachelor's degree and at least two years of related experience.
