Under the general direction of the Clinical Pharmacy Manager, the Clinical Pharmacy Specialists are responsible for the provision of safe, effective, and prompt medication therapy. Essential Responsibilities:
Provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting with physicians, nurses, and other healthcare providers. Facilitate pharmaceutical care services, direct patient care programs, and medication utilization systems within assigned care areas to ensure drug utilization aligns with patient needs, evidence-based practices, and regulatory standards. Design and implement stewardship activities and restriction/surveillance programs. Participate as an active member of a multidisciplinary team, making evidence-based, patient-centered medication recommendations. Manage medical emergencies as part of the team. Review discharge medications, perform reconciliation, and provide counseling as appropriate. Identify practice-related issues requiring evaluation and facilitate clinical projects and quality improvement initiatives to advance practice. Identify, design, and implement improvements in the medication-use system to enhance patient safety, therapeutic outcomes, or cost control. Maintain proficiency in preceptor roles and develop training experiences for students, residents, and staff. Facilitate specialty medication procurement, ordering, and dispensing procedures. Monitor patient profiles to identify and mitigate drug-related problems, improper drug or dose selection, adverse reactions, interactions, and treatment failures. Communicate effectively with healthcare providers and caregivers to ensure continuity of pharmaceutical care. General Responsibilities:

Education:
Graduate of an accredited pharmacy degree program. Experience:
PGY-1 Pharmacy Residency preferred or 3 years of clinical pharmacist experience. Licensure/Certifications/Registrations:
Licensure as a Doctor of Pharmacy by the Oklahoma State Board of Pharmacy. Preceptor certification when eligible. Board Certified Pharmacotherapy Specialist preferred. BLS certification and Immunization Administration Certificate preferred; BLS required if technician holds Immunization Certification. Knowledge, Skills, and Abilities:
Knowledge of hospital/clinical practice and services. Expertise in advanced pharmacy practice, specialty medication management, and evidence-based medicine. Excellent communication skills. Proficiency with Microsoft Office and EHR tools. #CB Current OU Health Employees - Please click
