Sun River Health provides high-quality comprehensive primary, preventative, and behavioral health services to all, regardless of insurance status and ability to pay, especially for underserved and vulnerable populations. We are a Federally Qualified, Non-Profit Health Center serving communities across Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia, and Westchester County. Currently, we are seeking a Part-Time Psychiatric Nurse Practitioner to join our team at our Riverhead, NY location. Role Overview
The Psychiatric Nurse Practitioner offers psychiatric clinical services, including medication management and evaluations, in compliance with organizational standards. Essential Duties and Responsibilities
Consult with MD/Psychiatrist/LCSW via eCW, telephone, and patient visits Manage medications and address behavioral health concerns in collaboration with physicians Provide clinical care to support care plan goals, with referrals as needed Participate in case conferences and high-risk meetings Approve treatment plans for patients in behavioral health care Support primary care and behavioral health integration efforts Coordinate transitions of care with external providers Respond to clinical team crises Conduct psychiatric assessments, diagnoses, and evaluations Contribute to diagnosis and treatment planning discussions Document reports and clinical notes accurately Collaborate with other mental health and medical staff Prescribe and monitor psychotropic medications Provide in-service training to staff Review case records for quality assurance Provide age-appropriate care, understanding growth and development principles Identify and address needs across different age groups Understand substance use disorder diagnostics and referrals Minimum Education and Licensure
Master's Degree in Nursing; Nurse Practitioner license in Psychiatry; CPR/BLS certification. Required Skills & Knowledge
Knowledge of psychiatric diagnostic and treatment procedures Competence in delivering healthcare and preventive education Ability to collaborate effectively within a team Bilingual in English and Spanish preferred About Sun River Health
