Job Summary The ALS House Call Program extends care from the multidisciplinary ALS clinic at the Healey Center for ALS at Massachusetts General Hospital, with a focus on research and innovative care delivery. Established in 2017, the program provides specialized home-based care, reducing medical isolation and enhancing communication. The Advanced Practice Provider (APP) will deliver personalized care at patients' homes, mainly in the MetroWest area, collaborating with clinic providers and integrating novel care techniques, including mobile health solutions, into patients' lives. The APP will work under supervision, performing home visits, developing care plans, and coordinating with community providers to improve patient experiences and continuity of care. Flexibility, independence, strong communication skills, and adaptability are essential qualities for the ideal candidate. Qualifications Principal Duties and Responsibilities: Conduct clinical home visits, primarily in the MetroWest area. Collaborate with healthcare providers to deliver targeted services to patients with motor neuron disease. Perform physical exams and preventive health measures. Develop and implement disease-specific management plans, document progress, and ensure continuity of care. Educate and counsel patients on treatment adherence. Order, interpret diagnostic tests, and evaluate clinical problems. Record findings, formulate plans, and prognoses. Prescribe or recommend medications and therapies. Provide home gastrostomy tube exchanges. Perform specific respiratory procedures at home. Complete ALSFRS-R assessments as requested. Conduct serious illness conversations and complete MOLST forms. Maintain accurate electronic medical records. Assist physicians and participate in program development and evaluation. Work with a patient services coordinator to implement the program, including virtual visits. Develop tracking systems to evaluate program effectiveness. Standardize telehealth and homebound care systems. Explore innovative care models and mobile health technologies to enhance patient care and reduce burdens. Coordinate with external facilities and providers for comprehensive patient care. Disseminate educational materials to patients and communities to improve care continuity. Support challenging patient/physician situations. Participate in network development and quality initiatives. Attend hospital and system committees as assigned. Report activities, patient satisfaction, and recommendations to leadership. Prepare reports on the program's performance for funding entities. Skills/Abilities/Competencies: Strong clinical and healthcare knowledge for effective communication. Exceptional organizational and time management skills. Motivated, independent, innovative, and adaptable. Excellent interpersonal, written, and oral communication skills, including medical terminology. Proficient in information systems and technology, database management, and reporting. Critical thinking and decision-making abilities. Ability to coordinate with diverse populations and handle sensitive information discreetly. Knowledge of safety, compliance, and regulatory standards. Licensure: Current licensure in Massachusetts. Education: Master’s Degree required. Experience: Previous APP experience required; neurology or palliative care experience preferred. Working Conditions: Regular travel by car for home visits in Massachusetts. Internet access required; work laptop provided.
