to support individuals in need—both in their homes and for local facilities. Whether you're a former family caregiver or a professional with dialysis training, your experience is invaluable.
What You'll Do:
Provide support during and after dialysis treatments, ensuring safety, comfort, and emotional reassurance
Assist with mobility, daily living tasks, and creating a clean, recovery-friendly environment
Monitor for post-treatment symptoms such as fatigue or dizziness and communicate concerns promptly
Support patient understanding of home dialysis protocols, diet, hydration, and treatment compliance
Collaborate with family members and facility staff to ensure continuity of care
Support transitional care for those newly discharged or beginning dialysis at home
Who You Are:
You've received dialysis training (e.g., through DaVita, Fresenius, or a home hemodialysis (HHD) program)
You've worked as a care partner, whether professionally or as a dedicated family caregiver
You're compassionate, detail-oriented, and confident assisting someone with complex care routines
You're comfortable using a mobile app (Herewith) to accept shifts, manage your availability, and communicate with clients
CPR/First Aid certification is preferred; CNA/PCT/HHA credentials are a plus but not required
