Hospice Patient Care Manager / Assistant Director of Nursing

We are seeking a compassionate and experienced Registered Nurse to join our team as the Hospice Patient Care Manager / Assistant Director of Nursing. This vital position oversees the implementation of our company's policies and mission, ensuring optimal staffing levels and high standards of care. You will work closely with the Hospice DCS / DON to facilitate smooth operation of hospice services and support our devoted care teams. Salary:

$95,000.00 - $110,000.00 per year

Essential Duties and Responsibilities: Supervise, orient, mentor, and continuously evaluate the patient care staff. Ensure compliance with all agency policies, procedures, and all applicable state and federal regulations. Participate in the Manager on-call rotation to handle emergent issues after hours. Oversee talent acquisition processes to maintain optimal staff levels. Coordinate and facilitate interdisciplinary team (IDT) meetings to ensure cohesive patient care planning. Contribute to quality assurance and performance improvement initiatives. Manage patient caseloads and direct the IDT process for multiple physicians and teams as needed. Minimum Position Qualifications:

Associate's Degree in Nursing is required; Bachelor's degree is preferred. Active and valid RN license in Texas. Minimum of 3 years of hospice care experience. At least 1 year of management experience in a healthcare setting. Valid Texas Driver's License and proof of auto insurance. Preferred Qualifications:

Certified Hospice and Palliative Nurse (CHPN) certification is preferred. Skills:
