The Intake/Home Care Coordinator is a key player in providing exceptional care by serving as the first point of contact for UnityPoint at Home. In this dynamic role, you'll manage incoming referrals through multiple channels—phone, fax, and electronic systems—while carefully reviewing patient details to ensure their individual needs are met. You'll enter critical patient data accurately in the EMR system and help coordinate smooth and timely admission visits, all within a fast-paced setting.
Your role requires frequent interaction with a variety of healthcare professionals, including hospitals, physicians, skilled nursing facilities, and care teams, as well as patients and their families. With your strong customer service skills, you will work closely with referral sources and internal teams to streamline the admission process and ensure our patients receive the best possible care when they need it most.

Responsibilities
· Maintains a broad knowledge in all UnityPoint at Home service lines including, Home Health, Hospice and Infusion services.
· Routinely monitors for new referrals via fax, EMR and other electronic applications.
· Accepts all new referrals and initiates referral documentation in the EMR and ensures accurate information and documentation is included in patient record to facilitate the admissions process, which includes diagnosis codes after validating with RN.
· Secures, confirms and accurately documents provider orders in a timely manner.
· Operates computer system to record/document and retrieve information to ensure appropriate agency services are provided in a timely manner.
Qualifications
Education:
HS Diploma or equivalent with successful completed course work
Minimum Experience:
Six months phone based customer service experience. Experience should include high volume, fast paced customer excellence environment such as a call center or similar phone based service role.
