Position Overview:
We are seeking a dedicated Licensed Practical Nurse (LPN) to join a skilled care facility in Queens, NY. This role focuses on providing high-quality long-term care to patients in a supportive and professional environment.
Key Responsibilities:
  • Provide direct patient care in a long-term care setting.
  • Administer medications and treatments as prescribed.
  • Monitor patient health and report changes to the healthcare team.
  • Collaborate with RNs, physicians, and other staff to ensure quality care.
  • Maintain accurate patient records and documentation.
Requirements
Qualifications:
  • Education: Practical Nursing Diploma or equivalent.
  • Licensure: Active New York State LPN license (required).
  • Experience: Minimum 1 year of experience in long-term care.
  • Technical Skills: Proficiency in skilled nursing care.
  • Soft Skills: Compassionate, detail-oriented, and strong communication skills.
  • Additional Requirement: Must be immune to Hepatitis B.
