Job Summary

The position of Medical Director serves as the senior clinical leader within our multidisciplinary nonprofit organization that provides comprehensive social services to individuals and families across diverse settings. This leadership role is responsible for overseeing and integrating medical services across residential programs, outpatient clinics, withdrawal management and medication for addiction treatment (MAT) programs. The Medical Director will ensure that clinical services are evidence-based, trauma-informed, culturally responsive, and aligned with the organizations mission to serve vulnerable and underserved populations.

Position is part time, and remote with the occasional requirement to be on site.

Primary Responsibilities

Provide clinical oversight and leadership for all medical services across residential, withdrawal management, outpatient, and medication for addiction treatment (MAT) programs. Ensure medical personnel follow medical policies and standards Develop, implement, and monitor medical protocols, policies, and procedures to ensure compliance with federal, state, and local regulations. (DMC ODS Source: 22 CCR 51341.1 (b) (28) (A) Collaborate with program directors and clinical teams to ensure integrated, trauma-informed, and culturally competent care across service areas. Supervise and support medical staff, ensuring high standards of care and professional development. Act as collaborating physician for NPs and PAs. (DMC ODS source: 22 CCR 51341.1 (b) (28) (A) (i) (a) Oversee withdrawal management program embedded in our SUD residential program to ensure the medical stability of persons served Oversee the administration and clinical quality of MAT services, including adherence to evidence-based practices for substance use treatment. Participate in interdisciplinary case reviews and provide direct consultation on complex medical and psychiatric cases as needed. Review each beneficiarys personal, medical and substance use history in all SUD OP programs This includes documentation of medical review in clients chart. This task can be delegated to a NP or PA, with oversite by Medical Director Review of H&Ps in SUD residential programs This includes documentation of medical review in clients chart. This task can be delegated to a NP or PA, with oversite by Medical Director Complete medical peer reviews of medical documentation on an annual basis. Participate in any County/network of care medical peer reviews of medical charts Completion of 5 hours of addiction medicine every calendar year. This role also supports staff training, contributes to quality improvement initiatives, and helps secure funding by participating in grant writing and reporting activities.

Additional Responsibilities Include

The Medical Director will be responsible for ensuring the delivery of high-quality medical services to clients and oversee the Medication for Addiction Treatment (MAT) program for alcohol, opioid, and stimulant dependence. This includes supervising assessment and treatment planning, prescribing medications as clinically indicated, and providing medical exams when necessary. The Director will review comprehensive client medical data such as health questionnaires, diagnostic test results, and various clinical reports to determine eligibility. They will also provide support for clinical workflows in collaboration with the medical team. Maintaining accurate and timely documentation in the Avatar system is essential. The Medical Director will serve as a backup provider and ensure continuous improvement of services by expanding MAT offerings to include long acting injectables and enhanced Alcohol Use Disorder (AUD) treatments. Additionally, they will guide professional development and support for Advanced Practice Clinicians (APCs) preparing for CARN-AP certification. The role also includes onsite Hepatitis C, HIV, and syphilis screening and testing initiatives.

Ensure quality of all medical services provided to clients. Oversee withdrawal management services; support of medical staff, providing consultation, reviewing cases and may include completion of physical exam of persons served within 72 hours of entering facility or the review of a physical exam that was completed within the last 12 months. Oversee Medication for Addiction Treatment (MAT) services for alcohol, opioid, and stimulant dependence. Serve as backup provider, including conducting assessments and developing treatment plans. Provide medical exams for potential recipients as needed. Review all relevant client medical data, including Encompass Health Questionnaire, Medical History and Physical Exam Form, Dependence Worksheet, COWS, CIWA, AUDIT, DAST assessments, SCHIE and CURES reports, Rapid UA, pregnancy, and breathalyzer test results Determine client eligibility for MAT services. Prescribe medications as medically appropriate. Collaborate with MAT team to ensure effective and consistent workflows. Maintain timely, accurate documentation in the Avatar system. Complete Continuing Medical Education (CME), exam prep Oversee APCs preparation for the CARN-AP certification exam. Lead expansion of MAT services to include: Long-acting injectable medications Enhanced treatment for Alcohol Use Disorder (AUD) Partner with Director of Nursing to implement onsite Hepatitis C (HCV) and HIV screening and testing.

Optional Additions

Provide medical and administrative direction and supervision to subordinate professional, technical and clerical staff. May cover treatment and diagnosis in the absence of the NP or PA. Evaluate and review the work of subordinate or other medical providers for appropriate diagnosis, treatment and chart documentation. Plan and prepare protocols and standardized procedures. Manage specialty contracts with specialty medical physicians. Maintain effective relationships with other County Departments, County policy makers and community health stakeholders Participate in the analysis, evaluation and planning of health or mental health programs. Conduct in-service training programs. Provide mental health court testimony on conservatorship if specializing in psychiatry. Manage division productivity to ensure that the budget targets are met.

Qualifications

Medical degree (MD or DO) from an accredited institution. Board-certified or board-eligible psychiatrist or physician or doctor of Osteopathic Medicine with relevant experience. Current, unrestricted medical license in the state of California and meets the requirements set forth in sections 51000.70 and 51341.1(b)(28) of Title 22 of the California Code of Regulations. Enrollment with DHCS under applicable state regulations and have been screened in accordance with 42 CFR section 455.450(a) as a limited category risk within a year prior to serving as a Medical Director and must have signed a Medicaid provider agreement with DHCS as required by 42 CFR section 431.107. Minimum of five years clinical experience, preferably in community health, behavioral health, addiction medicine, or child/adolescent services. Experience working with vulnerable populations, including those with co-occurring mental health and substance use disorders. Board certification in addiction medicine is preferred. Strong leadership, communication, and interdisciplinary collaboration skills. Knowledge of relevant healthcare regulations, including HIPAA, Joint Commission standards, and federal/state guidelines for MAT. Preferred ASAM-certified addiction medicine specialist, must be certified by one of the recognized certifying boards: the American Board of Preventive Medicine (ABPM), the American Board of Psychiatry and Neurology (ABPN) in Addiction Psychiatry, or the American Board of Addiction Medicine (ABAM).
