The work involves moderately complex to complex issues, requiring an in-depth evaluation of variable factors. The focus is on outpatient medicine, continuity of care, health maintenance, and disease prevention. Responsibilities include keeping medical histories, referring patients to specialists, and influencing departmental strategy. The role involves making decisions on complex issues with considerable autonomy. Job Responsibilities
Evaluate and treat patients according to standards of care Monitor care quality using data and chart reviews Coordinate patient services Participate actively in the care team through daily huddles Assist leadership in fostering a cooperative practice environment Complete medical records promptly, working with coding specialists Follow policies and protocols set by Clinical Leadership Meet with Regional Medical Director to review quality and compliance Participate in growth opportunities within the Center Ensure compliance with licensing, certification, and accreditation standards Qualifications
Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine (preferred) Active, unrestricted DEA license Medicare Provider Number 2-5 years of relevant experience (preferred) Experience managing Medicare Advantage panels and understanding of coordinated, value-based care Knowledge of Medicare guidelines and HEDIS quality indicators Additional Details
