Primary Care Physician (MD/DO)

to provide quality primary medical care to our patients in the comfort of their homes.

Position Overview As a Primary Care Provider, you will assess members with multiple comorbidities residing in the community. You will perform comprehensive evaluations of patients and provide medical management.

Responsibilities

Examine patients and order appropriate diagnostic workups

Assess members with acute condition changes promptly

Maintain complete and accurate medical records

Collaborate with interdisciplinary team to develop care plans

Educate patients on disease processes, medications, and compliance

Manage patient medical care

Qualifications

Licensure and current registration to practice as a Physician in New York State

Strong communication and follow-up skills

Proficiency with Microsoft Office, including Excel, and experience with data entry, documentation, and report generation

Experience with the geriatric population and home care preferred

Knowledge of medical documentation
