Job Summary The Data Integrity Analyst, EMPI directs the review and analysis of medical data records in clinical and non-clinical areas to complete the critical Enterprise Master Patient Index (EMPI) correction process. These responsibilities will include remediation functions, which include critical decisions regarding medical record merges, unmerges, patient data corrections, and medical record overlay corrections. Typically reports to Enterprise Master Patient Index/Data Integrity Manager, Health Information Management.

Job Description

MINIMUM QUALIFICATIONS Education: Associates or Bachelor’s degree in Health Information Management, Nursing, Information Technology or healthcare-related field preferred.

Licenses/Certifications: Registered Health Information Administrator (RHIA), Registered Health Information Technician (RHIT), LVN or RN preferred.

Experience / Knowledge / Skills: Four (4) years of experience in health information management or in a healthcare administrative, registration, information services, or other healthcare business role required with Associates or Bachelor’s degree. In lieu of a degree, six (6) years of experience in EMPI corrections and data integrity required.

Experience in chart review with an emphasis on data integrity.

Experience working with medical record applications.

Excellent computer skills including working in Microsoft Office programs (Excel, Word, Outlook, PowerPoint).

Effective oral and written communication skills.

Critical thinking skills, strong problem-solving skills, attention to detail, able to work independently and on a team.

PRINCIPAL ACCOUNTABILITIES Responsible for reviewing and analyzing the medical record and investigating of all clinical, non-clinical data and technical interface details to complete the critical EMPI correction process. These responsibilities will include remediation functions which include critical decisions regarding medical record merges, unmerges, patient data corrections, and medical record overlay corrections.

Works in various applications to identify and correct duplicate medical record numbers, overlays, and documentation data integrity issues.

Responds to service tickets, prioritizes them, and processes the incoming requests for medical record corrections in a timely manner and in consideration of patient care and safety.

Responds to questions received via emails, phone calls, and/or information service tickets regarding chart corrections.

Communicates with members of the data integrity team, management team, billing office, ancillary departments, privacy, information services, legal and clinical team when needed for resolution of EMPI issues.

Communicates with clinicians and patients.

Reviews, validates and merges duplicate medical records.

Performs EMPI remediation tasks for medical record duplicates and overlays.

Provides follow-up information on duplicates, overlays, changes and trends to departments and leadership. Informs manager on status of projects and assignments.

Makes document adjustments at both patient and encounter levels via corrections process and re-indexes documents when appropriate.

Maintains and updates EMPI correction log as appropriate.

Prioritizes and executes projects according to established schedules and timeframes.

Maintains an effective level of coordination on all assigned projects.

Trains new or less experienced staff to ensure knowledgeable use of applications in order to accurately complete the EMPI correction process.

Performs other duties and special projects as assigned by manager or team lead. Adheres to all MH policies, procedures, and standards, and within budgetary specifications.

Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
