Job Description
What You’ll Do
Conduct in-home and community-based health assessments Address chronic and acute care needs across adult, pediatric, geriatric, and women’s health populations Collaborate with patients, specialists, and care teams Participate in pop-up clinics and outreach events Travel daily within a 60-mile radius (up to 120 miles/day)
Schedule Options
Option 1:
Standard 8-hour days (Mon–Sat)
3 days: 8AM–5PM 2 days: 10AM–7PM 6–7 patients/day
Option 2:
4 x 10-hour days (Mon–Sat rotation)
8AM–7PM 8–9 patients/day One Saturday/month required
Job requirements
Minimum Requirements
Master’s degree in Family Health (ANCC/AANP accredited) Active & unrestricted NP license National board certification (ANCC or AANP) Valid driver’s license and willingness to travel EMR proficiency and strong documentation skills Basic Life Support (BLS) certification
Preferred Qualifications
2–5 years as a Nurse Practitioner Home or public health experience Bilingual fluency (preferred) Experience with immunizations and point-of-care testing
