Social Worker - Dementia Specialist
Eagle Crest Communities is seeking a Social Worker to joinour team of caring and compassionate individuals at our Riverside TransitionalCare facility.
What You'll Do Responsible for the identification of social,psychological, emotional strengths and assessment needs of each resident withintheir assigned caseload, and focus on behavior prevention, tracking and addinginterventions Promote adjustment to the total livingenvironment and respond to any resident and family questions or concerns Develop an ongoing discharge plan for eachresident within their assigned caseload, which shall include potential forreturn to the community and/or transfer plans for those requesting it Contribute to the interdisciplinary careplanning process, and contact community resources to organize for successfuldischarge to the least restrictive environment Utilize established best practices to educateemployees, residents and families on dementia care, as well as develop andimplement new material as approved by the Director of Social Services What You'll Need
Bachelor's degree in Social Work or relatedfield is required Currently licensed in the State of Wisconsin asa Social Worker One year post degree experience is preferred,but not required Ability to relate to and work efficiently withpersons who are ill, confused, disabled, and/or emotionally upset, as well aspersons of diverse backgrounds and educational levels Knowledge of medical, social, and geriatricterminology and principles Ability to relate collaboratively andprofessionally with other staff and community agencies Our Wages
