Community Health Lvn
The Community Health Educator Lvn possesses and maintains comprehensive knowledge about perinatal health, prediabetes, diabetes prevention, gestational diabetes, and management of Type 1 and 2 diabetes, Asthma, COPD, Chr and other chronic conditions. Develops and provides self-management education in the community and the hospital programs. Promotes and develops community engagement and education to promote equitable family engagement within the community. Will incorporate education principles that are current and reflective of evidence-based education and management. The posted compensation range of $27.77 - $40.27 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law. Unless directed by a collective bargaining agreement, applications for this position will be considered on a rolling basis.
