Healthcare United is seeking a Board-Certified Primary Care Physician (Internal Medicine or Family Medicine) to join our dedicated outpatient team in Chicago, Illinois . This full-time opportunity allows you to deliver high-quality, team-based care to an underserved adult patient population in a collaborative, patient-centered setting.
As a Primary Care Physician, you will:
Deliver comprehensive primary care to adult patients (ages 16+), with a focus on managing chronic conditions, behavioral health, and addiction.
Work collaboratively with an interdisciplinary care team, including behavioral health specialists, care managers, nurses, and support staff.
See 8 10 patients per day in a low-volume, high-acuity outpatient setting.
Rotate through a same-day clinic to provide timely care and prevent unnecessary hospital or emergency department visits.
Participate in a call rotation (1 week at a time), with low call volume (6 8 calls/week). No inpatient, rounding, or hospital responsibilities.
Leverage technology and EMR to support care coordination and patient outcomes.
Diagnose and manage complex adult patients with multiple comorbidities, addiction, and behavioral health concerns.
Prescribe medications, including opioids, when clinically appropriate.
Provide patient education and support self-management strategies.
Collaborate with support staff to develop and implement individualized care plans.
Contribute to quality improvement and population health initiatives.
Requirements
Current unrestricted medical license in the state of Ohio (Note: Practice is located in Chicago; licensure in Illinois will likely be required).
Board Certification in Family Medicine or Internal Medicine is required.
Board certification in other specialties considered on a case-by-case basis (Pediatrics and Occupational Medicine not applicable).
Minimum 2 years of full-time clinical experience post-board certification (residency or telehealth experience not counted toward this requirement).
DEA license and comfort with prescribing opioids.
Experience working with behavioral health and addiction medicine is highly preferred.
Prior work in an FQHC or Community Health Center strongly preferred.
Proficiency with Electronic Medical Records and care coordination tools.
