Job Description

The Psychiatric-Mental Health RN (PMH-RN) in Supportive Housing Services is a member of the interdisciplinary team which partners with individuals to achieve independence in their physical and mental health care needs. They provide health promotion, screening, maintenance, and education based on the individual's integrated health care needs. Assessment and monitoring of health care, case coordination, and self-care needs are part of the ongoing PMH-RN nursing process. Administration and oversight of treatment regimens as ordered by the prescribing provider. Support of non-clinical staff related to patient's medical/clinical needs. Management of all patient/client care needs related to medication, point of care lab, and infection control.

Essential Duties & Responsibilities
  • Interdisciplinary team member
  • Professional communication and collaboration
  • Provide medical/clinical education and support for staff
  • Provide life skills and wellness education and support for patients and families
  • Participate in the implementation of the Individualized Treatment Plan
  • Assist in facilitating linkage to health care services
  • Assist in discharge planning, as appropriate
  • Admission nursing assessments upon move-in to the group home, including:
    • General head-to-toe physical assessment
    • Medication Review
    • Allergy Review
    • Verify TB test results
    • Ensure any ordered homecare equipment is present and working properly (CPAP, O2, Ambulation Aides)
    • Ensure Primary Care and Psychiatric Care engagement
  • Medications:
    • Medication reconciliation and availability for self-administration
    • Medication Administration Record (MAR) management
    • Oversight and management of controlled medications
    • Administration of medications on site, as applicable
    • Medication disposal
  • Labs & Waived CLIA Testing:
    • Perform approved Point of Care (POC) testing, as ordered
    • Monitoring and reporting of lab results
  • Infection & Exposure Control:
    • Support staff with implementation of Infection & Exposure Control policies and procedures
    • Provide education and support to staff and patients/clients as needed:
  • Regulatory and compliance documentation and record maintenance
Professional Requirements

Meets dress code standards, appearance is neat and clean

Completes annual education requirements

Maintains regulatory requirements

Maintains and ensures patient confidentiality at all times

Reports to work on time and as scheduled; complete work within designated time

Represents the organization in a positive and professional manner in the community

Wears identification while on duty

Completes in-services

Attends staff meetings as assigned

Complies with organizational policies regarding ethical business practices

Communicates the mission, ethics, and goals of facility

Knowledge, Skills & Abilities

Effectively communicate via written, verbal, in person and virtual methods

Basic computer knowledge

Strong prioritization/organization skills

Qualifications
Education Level

High School diploma or equivalent. Graduation from accredited RN program.

Years of Experience

Required Certification/Licensure

Current Indiana license

Current CPR certification

Driving occasional; one RN per region; support of four group home locations in the central region, four group home locations in east region, and two group home locations in west region

Pay Range:

$29.63--$41.54
