Hospice RN Opportunity

This position is eligible for the Northern New Mexico differential of $4.00 per hour. (This differential may be adjusted or discontinued based on business requirements.)

Qualifications
  • Associate Degree in Nursing
  • Registered Nurse Licensure - Compact/Multi
  • Minimum one year in direct patient care setting or home health care.
  • Valid Driver's License
  • BLS required at time of hire
Responsibilities

Assess patient needs, including physical, mental, financial, and spiritual aspects

Provide skilled nursing care in alignment with physician/APC care plans

Coordinate patient care through collaboration with caregivers, multidisciplinary teams, and physicians

Adjust interventions as needed based on patient responses and updated orders

Evaluate effectiveness of interventions at each visit

Develop and implement hospice care plans while ensuring compliance with regulations

Maintain productivity standards for patient visits and caseload management

Communicate visit schedules and any changes to patients in a timely manner

Guide newly hired nurses and students, serving as a mentor and resource

Ensure documentation is accurate, timely, and compliant with agency policies

Delegate appropriate care tasks to LPNs and CNAs

Participate in quality improvement initiatives and performance improvement projects

Take on leadership responsibilities as required by the department

Provide holiday, weekend, and evening coverage as needed

Perform additional functions as required for patient care and agency operations
