Job Summary
The Social Worker plays a vital role in supporting patients and families through emotional, social, and practical challenges associated with illness, trauma, and life transitions. This position requires a compassionate, resourceful professional who can assess client needs, provide counseling, and coordinate access to community and healthcare services. The Social Worker also serves as an advocate for patients' rights and collaborates with multidisciplinary teams to ensure comprehensive care.
Key Responsibilities Conduct psychosocial assessments through interviews, case histories, and coordination with healthcare providers Provide counseling and emotional support to patients, families, and caregivers facing medical conditions, trauma, or significant life changes Connect clients with appropriate community resources, such as housing support, mental health programs, rehabilitation services, and financial aid Serve as a patient advocate, ensuring equitable access to healthcare, legal protections, and social support services Collaborate with nurses, physicians, therapists, and other professionals to develop and execute individualized care plans Maintain accurate and confidential documentation in compliance with legal and healthcare standards Support discharge planning, facilitating smooth transitions from hospital to home or long-term care settings Conduct crisis interventions in situations involving abuse, neglect, or acute mental health concerns
Required Qualifications Bachelor’s or Master’s degree in Social Work (BSW/MSW) or a related human services field Valid and current state licensure or certification as required (e.g., LSW, LMSW, LCSW) Strong interpersonal and communication skills Ability to manage a diverse caseload and work effectively under pressure Understanding of community resources, healthcare systems, and confidentiality laws
Preferred Qualifications Experience working in hospital, clinical, or behavioral health settings Bilingual (English/Spanish or other languages) is a plus Training in trauma-informed care, crisis management, or case management systems
Certifications (if any) State-issued license in Social Work (LSW, LMSW, LCSW or equivalent) Additional certifications in mental health, substance abuse, or case management (preferred but not required)
