Sr. Case Manager

Under the direction of the Senior Director of the Unhoused Care Team, the Senior Case Manager will provide compassionate person-centered, trauma-informed care for individuals experiencing homelessness. This position will focus on outreach, assessment, case planning, and navigation of supportive services to assist clients toward stability and permanent housing. This is a temporary position ending June 30, 2026, ideal for a final-year Master of Social Work (MSW) student or a professional holding an MSW who wants to make a direct and measurable impact in the community. Essential Job Functions and Duties 90% Conduct proactive outreach to individuals experiencing homelessness, establishing trust through consistent, compassionate, and trauma-informed engagement. Perform comprehensive biopsychosocial assessments to identify client needs, strengths, and barriers, and develop individualized service and housing stabilization plans. Facilitate access to a broad range of services, including housing assistance, physical and behavioral healthcare, substance use treatment, employment support, and public benefits enrollment (e.g., Medi-Cal, SNAP, SSI). Coordinate care and service delivery with multidisciplinary internal teams, external community-based organizations, and public agencies to ensure client-centered support. Provide intensive case management, including regular follow-up, goal tracking, and support in navigating complex service systems. Deliver crisis intervention, de-escalation, and safety planning as needed, ensuring timely response to urgent client needs. Advocate for clients' rights and access to services, including accompaniment to appointments, court hearings, or housing interviews as appropriate. Actively participate in case conferencing, team meetings, reflective supervision, and required staff training sessions. Support peer case managers and contribute to collaborative case planning when appropriate. Administrative Tasks 10% Maintain accurate, up-to-date, and confidential electronic records and case notes following DSDP policy, contractual obligations, and regulatory standards. Complete all required documentation, reports, and data entry in a timely and accurate manner to meet contract or funding requirements. Participate in program evaluation efforts, including data collection and client satisfaction surveys to inform service improvements. Adhere to all local, state, and federal guidelines and laws. (i.e., HIPAA, OSHA, FLSA, traffic laws) Adhere to all of the DSDP policies and procedures Required Qualifications, Skills, and Abilities

Valid U.S. Driver's License & clean driving record (preferred) Must be in the final year of a Master of Social Work (MSW) program or hold an MSW degree from an accredited institution. Comfort working with individuals experiencing mental illness, substance use disorder, and chronic homelessness. Demonstrated understanding of trauma-informed care, harm reduction, and housing-first principles. Ability to work varied hours, including weekends and holidays. Strong verbal communication, problem-solving, and organizational skills. Ability to work independently in the field and collaboratively within a team. Experience in homelessness services, social services, or community-based care management preferred. Ability to speak, read, and write in English and Spanish preferred. Pre-employment Requirements
