The Children's and Women's Health Navigation Coordinator (HNC) is a registered professional nurse who uses an evidence-based, multidisciplinary approach to support consumer navigation through the AdventHealth continuum of care. The HNC assists providers and multidisciplinary teams in navigating patient care pathways. They also assist leadership with program development by acting as liaison with stakeholder teams across the system, and providing resources/data. The HNC supports data collection, clinical collaboration, and marketing/community outreach efforts. They may also participate in process improvement activities to achieve shared campus goals. The HNC actively participates in outstanding customer service, and accepts responsibility for maintaining relationships that are equally respectful to all. The value you'll bring to the team: Collaborates with patient and healthcare teams in navigating patients needs (ranging from minimal to complex) through the healthcare continuum including, but not limited to, appointment coordination, record requests, insurance concerns, education, and communicating information between the physician office and the patient, etc. Maintains relationship with physician and clinical leadership throughout the AdventHealth System to better assist consumers including travel to other campuses, for education and collaboration, relationship building and to expand personal knowledge of network facilities and offerings. Assists leadership team with program development work, functioning as liaison with stakeholder teams, and as a resource for data and other department information. Completes accurate data collection (per direction of supervisor) and submits monthly reports as requested by leadership in a timely and efficient manner. Works with leadership and CHN teams to supports program awareness within the physician community and community at large both in person (may include travel) and virtually. Partners with consumer marketing team and participates in community events both in person and online. May include travel, public speaking, presentations, and attendance at off hour events. The Expertise And Experiences You'll Need To Succeed
Minimum qualifications: Bachelor's of Science degree in Nursing 1 year of nursing in outpatient setting experience Current RN license in state of Kansas Cardiopulmonary Resuscitation (CPR) Preferred qualifications: Behavioral Health Background Preferred Master's of Nursing 2 years of hospital care coordination experience Case Management certification ACM (Accredited Case Manager) CCM (Certified Case Manager) RN-BC (Board Certified in Case Management) This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
