Are you interested in a Float Pool opportunity? Join our dynamic WellStar Enterprise Support Team (WEST) and become a vital part of ensuring seamless staffing across WellStar facilities! As a member of the WEST Team, you'll have the exciting chance to float to various units based on the greatest need, with unit assignments determined by WFE leadership, considering your clinical expertise and skill mix.
Core Responsibilities and Essential Functions:
- Collaborate with physicians to establish and evaluate treatment programs, care plans, goals, and discharge plans.
- Develop a plan of care with input from the patient and/or family.
- Create a realistic discharge plan that incorporates patient and/or family goals.
- Implement physical therapy treatment plans.
- Select appropriate activities to advance patient goals.
- Revise treatment goals and plans based on patient and family input.
- Provide ongoing education and training to patients and families.
- Offer discharge instructions, follow-up, and referrals to community resources as needed.
- Act as a referral source for staff, families, and physicians regarding rehabilitation services and equipment.
- Document evaluations and care plans according to departmental guidelines.
- Record each treatment session as per departmental protocols.
- Complete and submit family education, discharge, and other required documentation in line with facility and program guidelines.
- Uphold ethical standards, demonstrating integrity and honesty.
- Embrace diversity and work collaboratively.
- Maintain patient confidentiality.
- Adhere to attendance and punctuality policies.
- Follow departmental dress code and display ID badge appropriately.
- Communicate effectively with team members.
- Participate in staff meetings, in-services, and process improvement activities.
- Organize workload efficiently and use materials and equipment safely and cost-effectively.
- Observe infection prevention precautions.
- Lead with vision, technology, innovation, and superior customer service.

- Engage in professional development and mandatory education.
- Complete core and annual competencies according to departmental timelines.
- Fulfill educational requirements to maintain licensure, including potential surgical training if interested.
- Participate in orientation and training for new employees.
- Complete all mandatory hospital and system requirements.
- Graduate from an accredited physical therapy program required.
- Doctorate in Physical Therapy required.
- Physical Therapist license required.
- Basic Life Support (BLS) certification from AHA required.
- Minimum of 3 years of acute or inpatient rehab experience as a Physical Therapist required.
- Effective communication skills.
