Physical Therapist (PT)
Summit Home Care and Hospice is searching for an experienced and motivated Physical Therapist (PT) to join our team. We're committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.
Physical Therapist (PT):
- Full-Time Hours: Monday Friday (40 Hours/Week)
- Annual Salary: $88,500 $104,000 (Based on a per visit rate + clinician's capacity for caseload volume)
- Coverage Location: Circleville, Ohio
- Expected Travel: 40 Mile Radius (Milage Reimbursement, $0.48/Mile)
Responsibilities:
- Develop and implement physical therapy programs that focus on rehabilitation.
- Work directly with patients to achieve maximum physical recovery.
- Evaluate effects of therapy treatment and communicate patients' progress.
- Record and document patient care services.
- Collaborate with other team personnel to achieve well-rounded care.
Requirements:
- Active Physical Therapy License & DPT from an Accredited Program, Required.
- Possesses & Maintains Current CPR Certification.
- Recent TB Test.
- Valid Driver's License & Reliable Transportation.
Qualifications
Qualifications:
- Previous experience in physical therapy, rehabilitation, or other preferred.
- Two years of appropriate experience as a physical therapist.
- Community/home health experience is preferred.
- Demonstrates good verbal and written communication and organization skills.
- Ability to handle physical workload.
- Ability to build rapport with patients.
- Strong leadership qualities.
