As a Licensed Vocational Nurse (LVN ), you will be an integral part of our team, providing high-quality nursing care and support to individuals with disabilities in our Intermediate Care Facilities (ICF). This role offers you the opportunity to work in a small, close-knit community where you can build meaningful relationships with both the residents we support and your fellow dedicated team members.
Licensed Vocational Nurse (LVN) Essential Responsibilities:

• Provide skilled nursing care to individuals with disabilities in an Intermediate Care Facility setting
• Implement and oversee the health care plans for residents
• Administer medications and perform medical procedures as needed
• Collaborate with a multi-disciplinary team to ensure the well-being of residents
• Maintain accurate medical records and documentation
• Offer support and guidance to Direct Support Professionals (DSP)
• Participate in complex case management and engage in continuous learning opportunities
  

Licensed Vocational Nurse (LVN) Qualifications:

• Valid License Vocational Nurse (LVN) license in the state of California
• CPR certification
• Strong communication and interpersonal skills
• A genuine passion for providing quality care and support to individuals with disabilities
• Ability to work effectively in a team-oriented environment
• Willingness to adapt to the needs of the residents and facility
