This role is crucial for providing high-quality patient care in a fast-paced hospital environment. The ideal candidate will have a minimum of two years of experience in a hospital setting and will be responsible for case management responsibilities, including discharge planning, care coordination, and utilization management. Key Responsibilities: - Collaborate with interdisciplinary teams to assess and plan for patient needs, ensuring effective care coordination.- Manage discharges, guardianship, and surrogacy planning while maintaining open communication with patients and their families.- Conduct assessments and develop discharge plans that consider long-term care placement when necessary.- Utilize the Expanse & Careport documentation systems for patient records and case management documentation.- Adhere to unit guidelines and policies to maintain standards of care. Required Qualifications: - Active Compact License (at submission)- Bachelor's in Social Work (BSW) or higher- Experience with inpatient case management- Familiarity with discharge planning and long-term care placement- Competency in working with families to navigate care options- Strong communication and interpersonal skills- Ability to manage a patient ratio of 25-40 Preferred Qualifications: - Previous experience in a fast-paced hospital environment- Knowledge of utilization management and revenue integrity procedures Work Environment: - The Case Management team consists of 3 Social Workers and 10 RN Case Managers, supervised by a manager and a director.- The department emphasizes collaboration and supports staff through integrated resources.
