Duration:13 Weeks Start:09/08/2025 Shift: Day 5x8-Hour (08:00 - 16:30)

Description: Provides counseling, support, and advocacy for individuals and families facing social, emotional, or health-related challenges. Conducts assessments, develops care plans, and connects clients to community resources. Collaborates with healthcare providers, agencies, and support networks to ensure comprehensive care. Maintains accurate documentation in compliance with legal and ethical standards

Must have active WA license to submit. Please indicate the WA license status : Active , in Process or no license on your profile. IF " in process status " Please provide applied date and ETA of issue date If your traveler has worked for any Providence affiliated facility as a perm staff - they must be separated from the health system for a minimum of 3 months to qualify for a travel contract. Open to MSW Care Manager (& have EITHER Associate clinical license or LCISW) or RN Care Manager. Requires a 1) Masters in social work and 2)Washington-issued post-MSW license such as the licensed social work associate independent clinical. (The associate licenses are the precursor to the "full" social work licenses.) Please confirm with submission clinician has the required docs. Must have case management and discharge experience. Utilization management experience is helpful but not required.
