Are you passionate about mental health advocacy? Our BH CP program has openings for a

Care Coordinator

to support the health, safety, and quality of life of clients through advocacy, teaching, and service planning. The team is fun, flexible, and diverse -- working together toward a common goal and sharing our strengths and perspectives. You'll work with care management by identifying goals and developing care plans. In this role, you will provide person-centered and recovery orientated care management and coordination.

Full-time: 40 hours

Schedule: Monday-Friday

Location: This role is community based, meaning staff will spend their time on site in the office, in the community visiting members, and have some remote work.

$5,000 salary differential for bilingual candidates that meet our members needs - bilingual Spanish, American Sign Language, or Haitian-Creole needed

What You'll Do:

Outreach and engagement with enrollees using telehealth and/or community visits Assist Enrollees to identify goals and to develop an individual care plan Complete comprehensive assessments Facilitate Care Teams Coordinate communication among Enrollees, their providers, involved state agencies and others Support Enrollees to access and utilize resources to improve and maintain their health and quality of life. What You'll Need:

Education: Bachelors in a behavioral health or social services related field. Skills: Valid driver's license and access to reliable vehicle required. Bilingual Spanish, American Sign Language, or Haitian-Creole preferred. Please indicate language capability in application. Additional salary differential for program needed languages! Experience: 1-2 years of experience with working with people with behavioral health disorders required; experience working with people with substance use disorders preferred.
