54 BROWNSBERGER CIRCLE, Fletcher, 28732 The role you’ll contribute: Social work practice consists of professionally responsible intervention to 1) enhance the development, problem-solving, and coping capacities of people, 2) promote the effective and humane operation of systems that provide resources and services to people, 3) link people with systems that provide them with resources, services, and opportunities, and 4) contribute to the development and improvement of social policy, generally and specifically health and welfare policy in the primary care setting and community. The focus of the social work assignment and intervention is on patients referred by a primary care provider or clinical staff due to at risk status or for unmet psychosocial needs. Serves as the subject matter expert for patient psychosocial and behavioral health needs, and social work practice for Shawnee Mission Physicians Group. The value you’ll bring to the team: Function as a treatment team member, interpreting social, psychological, emotional and family system problems and strengths to other members of the treatment team Prioritize clinical problems, formulate treatment goals and construct treatment plan, revising as needed, based on continuous evaluation and assessment of progress Incorporates patient, physician, customer needs and concerns into decision-making Demonstrate ability to provide social work services to patient, family members and significant others in four areas: 1) psychological functioning, 2) adaptation to illness, injury, disability or life event, 3) psychosocial assessment/evaluation and 4) facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge Identify and intervene for suspected abuse, including sensitivity to age specific concerns: physical/emotional/sexual abuse of children and adolescents: domestic abuse for adults; caretaker/institutional abuse for senior adults Manage crisis situations as needed, demonstrating ability to quickly appraise crisis situation and select appropriate intervention Document direct service in patient’s medical record after each significant encounter Demonstrate proficiency in use of the electronic medical record Adhere to principles and requirements of PCMH and CPC+ programs Display a high level of flexibility, adaptability and organization skills Participate in performance improvement projects and quality outcome activities Accept special projects as assigned Attend and participate constructively in staff meetings Assists in tracking and documentation of avoidable days Prioritize and organize time and functions Communicates effectively and respectfully with patients, vendors, and all levels of staff and management Provides conscientious commitment to assigned work, work schedule, honesty, teamwork, assisting others to bring about positive outcomes Recognizes and manages risks, resolves issues and problems as soon as they arise, reports and documents adverse events and reportable conditions, Verbally and behaviorally supports Shawnee Mission Health’s mission and values
