Pay $70,000 - $71,000 annually

The Sr. Habilitation Coordinator is a blended position which is responsible for designated leadership tasks while maintaining a reduced Habilitation Coordinator caseload. This position supports the HC department by completing assigned supervisory, training, mentoring, and quality review functions in collaboration with and in the absence of the Director of Habilitation Coordination.

Core Responsibilities

Activities include but are not limited to the following:

Completion of quality documentation checks for assigned records / caseload to ensure compliance with agency policy & procedure and in accordance with regulatory requirements including, but not limited to: Monthly note reviews and follow up (training, coaching) Electronic Record - Staff Action Plans /Service Plans/ IPOPs/ Safeguards review and follow up (training, coaching)

New staff training in collaboration with Director, including providing Documentation Training in new hire orientation Facilitate department meetings / take minutes Mentor, coach, new Habilitation Coordinators for first six months of hire Maintain clear and open communication with Director of Habilitation Coordination and ensure s/he is aware of any and all concerns with service delivery, compliance, or work performance of habilitation coordinators Directly supervisor HCs in Director’s absence Serve as Treatment Team Leader, coordinate services and supports with all members of the treatment team (Nursing, Behavior, Therapy) Develop, coordinate, and monitor each resident’s Person-Centered Program Plan. This includes all programs under HCBS Waiver (residential habilitation, day habilitation and residential community habilitation). Train staff on all service plans and associated documentation Ensure implementation and integration of services across the individuals’ programs Monitor the services provided to individuals through direct observation in each service setting. Complete monitoring visits during critical hours when people are in their home, i.e. evenings, weekends Complete daily/ weekly / monthly quality assurance reviews of residential, day and Residential community habilitation staff action plans and associated documentation. Provide follow up as needed with staff on documentation, service delivery, or other issues as they arise. Maintain program participants’ active charts, inclusive of the electronic record system, ensuring documentation is current and supports regulatory requirements. Participate in leadership activities including meetings, trainings and committee involvement. Ensure positive and effective communication and cooperation within and between department, division, agency, and community. Serve as point person, liaison with people’s family and other natural supports Establish a work environment that creates positive communication between supervisors and employees and assimilates new employees to the Agency’s culture, values and mission. Ensure compliance with all pertinent government and agency regulations and operating standards Qualifications

Bachelor’s degree required in Psychology, Sociology, or other related Human Services field. At least one year of experience with people with developmental disabilities Must be a Qualified Individual Disability Professional (QIDP). Basic computer skills (Windows, Microsoft word, excel, adobe acrobat, Outlook, internet & email; ability to utilize department software programs). Must have excellent organizational and communication skills Travel is required. Must have valid NYS Driver’s License. Must possess the ability to make independent decisions when circumstances are warranted. Must possess the ability to work independently and manage priorities in a mobile environment. Work Environment

Job related tasks involve exposure to blood, body fluids, tissue or the potential for skin or mucous membrane contact from spills or splashes of these substances. May have exposure to unpredictable individuals and situations when working at Upstate Cerebral Palsy sites. OSHA Exposure Category Working Conditions

Must have fluency in the English language and while performing the duties of this job the employee is regularly required to talk or hear. The employee frequently is required to stand, walk and sit. Must be able remain in a stationary position for extended periods of time. Ability to lift up to 20lbs and/or assist individuals in their care. Constantly operates a computer and other office productivity machinery, such as a copy machine, scanner, computer printer, etc. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
