Presbyterian is seeking a Transfer Center Nurse Specialist
The Transfer Center Coordinator is responsible to coordinate/inpatient bed assignments for critical care patients at Presbyterian. The Transfer Center Coordinator coordinates, prioritizes, and optimizes patient flow activities throughout the hospital network, as well as coordinates and facilitates the patients entry into Presbyterian from other health care facilities

Associates degree in nursing AND 2 years of clinical nursing experience required.
New Mexico Board of Nursing RN License required.

Coordinates and facilitates the transfer of patients from other health care facilities to Presbyterian Hospital using Transfer center process flows.
Collects clinical information to determine patient s level of care
Collaborates with Presbyterian on-call physicians to arrange acceptance by the attending physician specialist (uses intranet paging system)
Communicates with referring hospital to coordinate transportation arrangements and provide an assigned bed
Bed assignment to all patients in Hospital Computer System and electronic bedboard
Accurately completes Transfer Center Database or delegates to Transfer Center Scheduler
Participates in evaluation process by faxing evaluations to other health care facilities
Ensures safe and timely placement of patients based on admission status, physician requested level of care, patient s clinical information and nursing units admission discharge criteria.
Collects clinical information from patients, patients families, nurses, physicians and/or physicians offices to determine level of care for patient placement into an inpatient bed
All patients are assigned beds in Hospital Computer System and electronic bedboard, except for Mother Bed Division
Directs Transfer Center Scheduler or Bed Management Scheduler to assign beds to patients transferring out of critical care units
Edits all patients requests to reflect accurate patient information, admit sources and select point personnel pagers
Use PreAdmitTracking comment section appropriate to communicate discharge times and transfer unit information
Collaborate with PFC, nursing supervisor, Operations Coordinator, Manager/Director of Patient Logistics, regarding patient placement issues.
Registration
Responsible for accurate and timely completion of patient registration and account data into the Hospital Computer System.
Maintains 90% (or better) compliance accuracy rate with critical elements in account Registrations (a) Verification of demographic, guarantor and Insurance information
Uses the account documentation process in the hospital s registration system to document missing information Complies with all Revenue Cycle Education Policies
Responsible for ongoing detailed knowledge of the hospital computer system
Actively supports Presbyterian Hospital values as listed in performance evaluation.
Functions as preceptor for new employees, students and other hospital.
Provides shift to shift report regarding bed availability, elective list, ED holds, transfer center patients and difficult patient placement and decedent affairs issues.
