About the Role:
The Community Health Worker plays a vital role in bridging the gap between healthcare providers and the community by delivering culturally competent health education and outreach services. This position focuses on empowering individuals and families to improve their health outcomes through education, advocacy, and support. The role involves building trusting relationships within diverse communities to identify health needs and connect people to appropriate resources and services. Minimum Qualifications: High school diploma or equivalent. Experience working in community outreach, health education, or a related field. Strong communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Basic knowledge of healthcare systems and community resources. Preferred Qualifications:
Certification as a Community Health Worker or related credential. Bilingual proficiency, especially in languages prevalent in the community served including Spanish and/or Khmer. Experience working with underserved or vulnerable populations. Familiarity with data collection and reporting tools. Training in cultural competency and health promotion. Responsibilities:
Conduct outreach activities to engage community members and raise awareness about health programs and services. Provide culturally sensitive health education and information tailored to the needs of diverse populations. Assist individuals in navigating healthcare systems, including scheduling appointments and accessing social services. Collect and report data on community health needs and program outcomes to inform service improvements. Collaborate with healthcare providers, social workers, and community organizations to coordinate care and resources. Advocate for community members by identifying barriers to care and facilitating solutions. Participate in training and professional development to stay current on health topics and outreach strategies.
