Human Services Program Specialist

This is a Human Services Program Specialist position in the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) HIV/AIDS Program. Work is performed under the supervision of a Senior Human Services Program Manager. This position is responsible for conducting the HIV/AIDS 500/501 trainings and supporting the programmatic activities of the HIV/AIDS and Early Intervention Programs. This position has county-wide responsibilities to provide technical assistance to all FDOH CHD sites, Centers for Disease Control Prevention HIV grant funded agencies, and agencies with Memorandums of Understanding and/or Agreements as a Department representative. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. This position travels to make community visits as needed. This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHO) Information Security Policy, Protocols and Procedures. This position may view, update or release information, and has access to the following sets of confidential information: Clinical Information, HIV/AIDS Case Reporting and Surveillance, and STD Case Reporting and Surveillance. This position has Health Management System (HMS) access for the purpose of entering Employee Activity Reports (EARs), and HMS Electronic Health Record (EHR) access for entering client information, scheduling appointments, and for viewing all other clinical data including but not limited to scheduled appointments, client demographics, insurances, medications, labs, and provider assessments. This position has access to Surveillance Tool and Reporting System (STARS), Ryan White Part A & B databases, CAREWare and Provide Enterprise access for the purpose of entering service notes and viewing client eligibility status and information utilized to verify client's linkage to care and referrals to essential support services. Serves as a trainer and coordinator for the HIV/AIDS 500, 501, 501 Update and Train-the-Trainer. Provides HIV/AIDS, PrEP and Sexually Transmitted Diseases presentations including but not limited to other in-services and trainings for health department staff, community members and organizations/agencies, as needed. Performs HIV Counseling, Testing and Linkage (CTL) Early Intervention Program duties, but not limited to, conducting HIV counseling and testing and drawing blood for specimen collection. Responsible for ordering, disseminating and tracking of condoms, HIV CTL supplies and promotional/incentive items. Participates in quality improvement activities, assist FDOH-CHD's HIV/AIDS Program with quality improvement, quality assurance and technical assistance, as scheduled or requested. Performs technical assistance site visits and capacity building opportunities to County Health Department sites, registered HIV test sites in Palm Beach County and the community by increasing the knowledge and awareness of the scope of the HIV epidemic in Florida, HIV testing guidelines, legal issues, and resources for biomedical interventions. Builds and maintains successful working relationships with FDOH staff and community partners, including CBOs, ASOs, FBOs, PBSO staff, community health clinics, and FDOH Palm Beach County clinical staff, care coordinators and patient care specialists. Prepares and submits accurate and thorough reports. Maintains documentation of client linkages and referrals. Analyzes data to measure the effectiveness of HIV CTL programming activities to identify the resources or adjustments needed to meet forecasts for programming on a long or short-term basis. Develops policies, procedures, standards and rules, according to CHO guideline. Monitoring compliance reviews and quality assurance techniques for measuring quality of service delivery to ensure maximum efficiency. Analyzes HIV testing site data to create and provide information to providers detailing the HIV testing and prevention program activities. Assists in the planning, coordination and evaluation of testing related events including, National HIV Testing Day, World AIDS Day, and National days of observance in collaboration with the Communicable Disease Program. Represents FDOH-CHD at all meetings, in-services and trainings as assigned. Participate in workgroups, conferences and outreach activities, as needed. Perform other duties as assigned. Required Knowledge, Skills, and Abilities: Knowledge of HIV/AIDS, epidemiology, communicable diseases and current related issues. Knowledge of and experience with HIV Testing, Counseling, Referral and/or Linkage Services. Skill to interview clients in an efficient, effective, culturally sensitive and empathetic manner. Ability to interview clients, build a rapport, identify client needs and link client to needed services and follow up with clients. Skills in establishing and maintaining effective working relationships with organizations, groups, and individuals. Knowledge of the methods of data collection, analysis and report generation. Skill to compile, organize, and analyze data. Ability to communicate effectively verbally and in writing. Skill in electronic communication, to include utilizing Microsoft Office Outlook, Word and Excel, and other software applications. Skill in public speaking, including facilitating classroom training. Other job-related requirements for this position: Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. A Valid Driver's License. HIV/AIDS 500/501 training is required. Working hours: Daily from 8:00AM to 5:00PM. Total hours in workweek 40. This position is assigned to perform field work and may be required to work a flex schedule, to include evening and weekends, as program needs dictate.
