Overview: JCCA is hiring a Case Planner II to support families in crisis within our Preventive Services Program. This role provides more clinical and therapeutic support to families using the Solution-Based Casework (SBC) model, which integrates relapse prevention and solution-focused strategies to strengthen family systems and reduce the risk of child welfare involvement. Key Responsibilities: Provide individual and family counseling to support family stability Conduct clinical assessments and develop intervention plans Monitor progress and provide therapeutic support under the supervision of an LCSW-R Collaborate with an interdisciplinary team including social workers, psychologists, and psychiatrists Ensure all contact and documentation meets ACS and DSS standards Participate in case conferences, inter-agency meetings, and client planning sessions Conduct home visits and contribute to group activity planning Qualifications: Master's degree in Social Work (MSW), Mental Health Counseling (MHC), or Marriage and Family Therapy (MFT) from an accredited program Experience or knowledge in therapeutic modalities (individual, family, group) Understanding of trauma-informed care and child welfare systems Excellent written communication and clinical documentation skills Must demonstrate the ability to work with culturally diverse populations Spanish fluency highly preferred Hourly Range: $32.04

$33.34 per hour Work Environment: Hybrid (some remote work available)
