Social Work Program Coordinator

The Social Work Program Coordinator position consists of being the Care Coordination and Integrated Case Management (CCICM) Program Coordinator and providing direct patient care as a Social Work Lead Coordinator as assigned across the New Mexico VA Health Care System (NMVAHCS). Responsibilities include: Developing and implementing policies and procedures specific to NMVAHCS and ensuring compliance with local facility, VA directives, and other accrediting bodies and regulatory standards. Collaborating with nursing counterparts for the planning, development, implementation, monitoring, and evaluation of the CCICM operations. Planning, organizing, and directing administrative, operational, Veteran, and personnel activities for the service. Identifying cohorts of Veterans across NMVAHCS with the highest opportunity and highest need. Identifying potential areas where care coordination is taking place. Integrating inpatient and outpatient care coordinators, care managers, and case managers for seamless continuity of care. Ensuring compliance with policies and standard operating procedures for evaluating and mitigating suicide risk. Conducting psychosocial assessments to determine the needs of veterans and family members. Participating in treatment planning process, including discharge planning with other disciplines. Providing case management to veterans and families throughout the continuum of care. Providing a full range of social work services within commonly accepted standards of social work practice. Qualifications: Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Proficient in spoken and written English as required by 38 U.S.C. 7403(f). Hold a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduated of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-12 Social Worker (Program Coordinator): Licensure: Must be licensed or certified at the advanced practice level and able to provide supervision for licensure. GS-12 ASWB Examination: Must have completed and passed the Association of Social Work Boards (ASWB) advanced generalist or clinical examination. GS-12 Social Worker (Program Coordinator) Experience: Must have at least one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. GS-12 Social Worker (Program Coordinator) Knowledge, Skills and Abilities: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
