Job Summary: Responsible for the planning, coordination, and implementation of therapeutic activities for the participants of Senior TLC in a home and community-based model of care. Reports directly to the Adult Day Health Coordinator, Center Manager or designee.
Specifications
Education : High School Graduate or GED minimum and current
certification/licensure
in activities. Experience : Minimum 1 years’ experience in social or recreational program providing and coordinating services for a frail elderly population within the past 5 years. Number and Type of Employees Supervised (optional) : None Licensure, Registry or Certification Required : National and/or NC Activity Director Certification. Special Training : Must be able to work effectively in a team environment. Good understanding of psychosocial problems of aging. Meets a standardized set of competencies for the specific position description established by Senior Total Life Care and approved by CMS before working independently. Immunizations:
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact Ages of Patients Rendered Care: Neonate/Infant Early Childhood Adolescent Adult Geriatric All Age Groups Key Responsibilities : (*denotes an age-related skill or task) Participates as a member of the Interdisciplinary Team (IDT)/Care Team. Conducts an initial assessment and scheduled reassessment of all participants, or with a significant change in condition and assessment must be completed prior to scheduled team conference and/or care planning. Communicates participant changes with team members. Records, signs, and dates documentation in the participant's medical record in a timely manner as outlined in the STLC Documentation policy. Helps to develop, implement, and supervise the Activity Program in the center. Plans and provides both group and individual activities. Develops specific programs as part of the plan of care to include therapeutic and/or social activities. Serves as a resource to staff and coordinates volunteer activities at the center. Builds within the Center a sense of community and a warm social atmosphere in which participants are able to identify a support system. Plans a monthly/daily calendar for the Adult Day Health Center (ADHC)/Social Center based on participant interest and in collaboration with the Center CNAs. Participates in Quality Improvement (QI) activities as appropriate. Coordinates with various community resources. Supports team members and functions as a team player while providing that support and adheres to the “do no harm” team contract statement. Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care. Other duties as assigned.
