Summary: The Inpatient Hospice Nurse, RN is responsible for providing skilled nursing care to patients requiring continuous inpatient nursing care. Rotates weekends and works weekend shifts as needed.

Qualification Requirements Registered Nurse currently licensed to practice nursing in the state of North Carolina Minimum of two-years’ experience as a professional nurse, preferably in an oncology/acute care setting

Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors. Conducts the admission of the patient to the center by: (a) obtaining a medical history, particularly as it relates to the present condition and co-morbidities, from the patient/family member(s), (b) performing a physical assessment of the patient, (c) assessing the patient's support systems to determine what assistance will be available from family members in caring for the patient after discharge, (d) assessing the patient and family member(s) to determine what health teaching will be required, (e) assessing the patient's physical, psychosocial, spiritual needs and support systems to determine level of care needed, (f) explaining nursing and other services of the center to patients and families as a part of planning for care, (g) explaining the inpatient facility is a restraint free facility, and (h) smoke free facility and campus. Provides comfort to patient/family to facilitate quality of life and death with dignity: (a) manages and provides skilled nursing care as outlined in the physician's plan of care, according to the agency and the inpatient facility and as allowed by the Practice Act of the N. C. State Board of Nursing; (b) develops, initiates, and updates an individualized plan with appropriate goals in cooperation with other agency professionals, and paraprofessionals who will be providing care to the patient, (c) initiates preventive nursing procedures, as appropriate for the patient's safety, and (d) observes signs, symptoms, and reactions to treatments and drugs, and changes in the patient's physical or emotional condition; reports these changes to the physician, as necessary. Accurately clocks in and out using ADP. Completes electronic documentation every shift: (a) ensures complete documentation on an ongoing basis supporting Hospice guidelines. (b) checks for any outstanding documentation daily and completes prior to end of shift. Teaches and counsels the patient/family regarding: (a) nursing care needs, (b) related problems with the terminally ill patient, (c) preparation for death, and (d) discharge plans. Supervises the care given by the Certified Nursing Assistant to assigned patients by: (a) provides the certified nursing assistant specific duties he/she is to perform for each individual patient, (b) provides on-going supervision and evaluation of the care given by the certified nursing assistant, and (c) participates in periodic conferences with the Hospice Inpatient Director of Clinical Service, RN concerning the certified nursing assistant's performance. Maintains regular communication with supervisor: (a) communicates problems of the patient(s) on a timely basis following chain of command, (b) communicates information using current process and technology available to 3HC, (c)coordinates hospice care with the Hospice Inpatient Director of Clinical Service. Stays abreast of overall patient care and improves nursing care skills: (a) participates in monthly staff meetings at least 50% of the time, hospice's quality assessment and performance improvement program, the interdisciplinary team in the development and implementation of the patient/family care plan, patient care conferences, record audits, utilization reviews, quality improvement activities, and in-services; (b) attends and participates in interdisciplinary team conferences at least 75% of the time as requested; (c) attends agency mandatory in-services, hospice sponsored in-service

training/continuing

education seminars and satisfies CEU requirements of the State Board of Nursing; and (d) strives to improve nursing care through continuing education, active participation in professional and related organizations and individual research and readings. Demonstrates an interest in personal and professional growth: (a) attends and participates in workshops, mandatory in-services, seminars, and webcasts to keep abreast of current changes in rules, regulations relating to job functions; and (b) does individual reading and research. Demonstrates a willingness to be cost-effective in the use of Agency resources, the monitoring of waste, and the proper and safe use of supplies and equipment. Adheres to 3HC's Personnel Policy and performs other performs other duties as assigned by supervisor to promote the successful operations and future growth of 3HC.
