Penick Village Inc. Continuing Care Retirement Community is looking for an experienced. knowledgeable and efficient RN, MDS Coordinator to join our healthcare team. The ideal candidate for this position requires a keen understanding of advanced MDS with a focus on leading effective care assessment of Residents in a compassionate PDPM service excellence environment. Primary Responsibility: The primary purpose of the MDS Coordinator position is to complete the minimum Data Set (MDS) for all nursing residents according to Health Care Financing Association (HCFA) regulations and to complete Personal Care Physician Authorization and Care Plan on personal care resident. Along with MDS completion develop, implement and maintain/update individualized plans of care. Job Duties and Responsibilities: According to CMS regulations, completes the Minimum Data Set (MDS) for all skilled nursing residents. Performs electronic submission of data monthly within assigned time frame. Performs nursing assessment; develops, implements, and updates individualized plan of care. Obtains necessary information to plan, implement, and evaluate nursing care plan. Must be able to work full-time hours, plus extended work hours when demand requires. Willingness to work in other positions (within scope of practice) when necessary. Interprets operating procedures and policies of nursing department. Directs the care planning meetings weekly as scheduled. Ability to travel locally and to out-of-town workshops and seminars Reports any non-compliance of MDS disciplines to Director of Nursing. Must demonstrate ability to prioritize goals, work independently using good judgment, function within the care plan team to promote continuity of care, display positive attitude towards the functions of the facility. Creates and prints Care Plan schedule quarterly and distributes to team. Sets up and conducts resident/family conferences quarterly and/or as desired. Distributes and collects nursing documentation relevant to care plan process. Attends meetings and in-services as scheduled. Takes on-call as scheduled. Participate in surveys (inspections) made by authorized government agencies. Work with the Admissions team to review referrals for long term and short term stay. Input admitting diagnosis in Matrix Care.
Qualifications: Experience in geriatric nursing and management preferred. Must possess a current, unencumbered license to practice as a RN/LPN in this state. Must be able to read, write and understand the English language. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. Must be knowledgeable of nursing and medical practices and procedures as well as laws, regulations and guidelines that pertain to long-term care. Able to read and write English and accurately document observations. Able to verbally communicate with residents, families, and co-workers. Must possess strong clinical and leadership skills, a high level of initiative, and a high ability to problem-solve. Must possess strong assessment and organizational skills. Ability to adhere to personnel policies, safety policies, and residents' rights. Ability to meet attendance policy. Exhibits a concrete understanding of PDPM and its requirements for accurate coding. Educational Requirements: Graduate of accredited School of Nursing. Current RN/LPN License. Currently licensed as an RN/LPN in North Carolina. Graduate of an accredited RN/LPN program. Meet continuing education requirements to maintain RN/LPN License.
