Physical Therapist, PT (Healthcare)
We are seeking a dedicated Physical Therapist to join San Francisco Post Acute! We offer Full Time, Part Time, and PRN opportunities within our in-house therapy program.

$55

Job Responsibilities:
Develop effective treatment plans and obtain approval from referring physicians.
Treat patients according to the approved treatment plans.
Assist nursing staff with training Restorative Aides.
Supervise Physical Therapy Assistants in patient care, adhering to state practice acts.
Communicate patient progress and issues with supervisors and healthcare team members.
Participate in Patient Care Conferences, Utilization Review, and Rehabilitation Meetings.
Conduct in-services for staff training.
Maintain documentation including evaluations, treatment notes, progress reports, and recertifications as per state and third-party requirements.
Update treatment records in accordance with policies.
Educate patients’ families and nursing staff on maintenance programs and caregiver training for discharge preparation.
Participate in discharge planning.
Recommend and order necessary durable medical equipment to promote patient independence, such as braces, walkers, and grab bars.
Report equipment issues to ensure proper maintenance.
Maintain positive and professional relationships with patients, families, staff, and colleagues.
Minimum Requirements:
Valid Physical Therapist License
