OPEN POSITION:
Nurse Practitioner / Physician Assistant - Geriatric Primary Care
SCHEDULE:
- Full-Time
- Monday to Friday
- 8:00am to 5:00pm
- No Evenings
- No Weekends
- No Holidays
- Minimal On-Call
COMPENSATION:
- $110,000 to $130,000+ Annual Salary, negotiable dependent upon experience

LOCATION:
Fargo, North Dakota
COMPANY PROFILE:
Since its founding in 1981, this organization has been dedicated to serving seniors in compassionate, comfortable environments - including bringing care to them wherever they call home. Their services include primary care visits, hospice, palliative care, and more. They are currently hiring in their primary care practice, which is largely facility-based with limited house calls.
POSITION DESCRIPTION:
The new nurse practitioner or physician assistant (APP) will serve as the primary care provider for a dedicated panel of geriatric patients. Most patients are located in facilities - ALFs / ILFs / group homes - with some in private homes.
Providers see an average of 8 patients per day or 40 patients per week.
Specific duties of the role include, but are not limited to:
- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions
- adjust treatment plans as necessary
- monitor vital signs and patient responses to treatment
- educate patients and families on health management, disease prevention, and treatments
- provide guidance on lifestyle modifications and self-care practices
- work with physicians, nurses, and other professionals to coordinate comprehensive care
- refer to and work with hospice when appropriate
- ensure compliance with healthcare regulations and standards
The practice uses the CareLink EHR.
On-call is phone triage only with very low volume. Providers rotate the on-call responsibility, with a shift once per month. On-call activity is compensated beyond regular pay.
Providers receive ample support with charting and administrative work, utilizing efficient technology. Fridays have allocated administrative time if needed.
REQUIREMENTS:
- North Dakota APP license (or ability to obtain)
- Minnesota APP license (or ability to obtain)
- DEA
- advanced practice experience preferred
- new grads with RN backgrounds in home health, trauma, or ICU are encouraged to apply
