Join a supportive rehab team at a respected facility and make a lasting impact in a community-focused setting.Job Responsibilities: Evaluate and assess patients to develop individualized treatment plans Implement evidence-based physical therapy interventions to improve mobility, strength, and function Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care Educate patients and caregivers on exercises, safety, and rehabilitation progress Accurately document patient progress and maintain up-to-date medical records Utilize appropriate modalities and equipment to support therapy goals Monitor patient responses and adjust treatment plans as neededEducation & Certification Requirements: Active Physical Therapist (PT) license in the state of Georgia Doctor of Physical Therapy (DPT) or equivalent from an accredited program Minimum of 1 year of recent clinical experience preferred BLS certification required (AHA preferred) Strong interpersonal and communication skillsWhy Work with Connected Health Care?
