Hamilton Health Center (Hamilton), established in 1969, is a Federally Qualified Health Center (FQHC) located in Harrisburg, PA. Retirement Plan with matching opportunity Employee Assistance Program Job Summary: The Workforce Case Manager assists individuals who are motivated to achieve employment by providing case management and career coaching. This role involves developing personalized plans, offering job readiness training, and connecting clients to supportive services to help reduce barriers to employment. Essential Functions: Develop individualized, client-centered goal plans focused on employment, housing, transportation, and financial literacy. Assess client skills, interests, and employment goals to create tailored workforce plans. Provide ongoing coaching and support, including assistance with job applications, resume development, and interview preparation. Maintain accurate and timely case notes and program data; prepare monthly and quarterly reports. Track client progress for one year of employment, conducting regular follow-ups. Collaborate with partner organizations and participate in case management meetings, community events, and workforce development networks. Minimum Education / Work Experience: - Experience in human services, case management, or workforce development is required. - Cultural competence and the ability to work effectively with diverse and underserved populations. - Strong organizational, interpersonal, and communication skills. - Proficiency in Microsoft Office, Google Suite, and case management databases. - Ability to work independently in a dynamic environment. - A valid driver's license is required; bilingual candidates are preferred.
