Conduct telephonic outreach to schedule Medicare Annual Wellness Visits, primary care, and preventive care appointments. Assist patients with chronic disease education, goal setting, and health assessments under the supervision of a licensed nurse. Use standing orders to help complete preventive care measures like screenings and routine labs. Coordinate care and resources to ensure optimal patient outcomes. Qualifications
High School Diploma or GED Completion of an accredited Medical Assistant program Proficiency in computer skills, including databases, Excel, and Word Excellent communication skills, both written and verbal Experience in clinical or hospital settings and knowledge of disease processes Registered or nationally certified Medical Assistant with regular recertification BLS certification Preferred Qualifications
Experience with EPIC or Electronic Medical Records Knowledge of disease prevention and management Additional Details
