Nurse Navigator (Onsite-40 Hours)

Overview
ONSITE POSITION: This position is full-time, 40 hours per week, working Monday through Friday from 8:00am-4:30pm. The Nurse Navigator serves a designated population with responsibilities for assessment, planning of care, monitoring services delivered, and evaluating collected data for outcome information. The incumbent will ensure the provision of high-quality, outcome-oriented care to individuals and their caregivers across Kennedy Krieger Institute programs and departments. Responsibilities
Assess referrals using a standardized tool, patient and caregiver interview, and medical record review. Explain clinical referrals, treatment recommendations, and health literacy support to the patient, and answer any questions from the patient and/or caregiver. Ensure referral follow through by contacting the necessary referral departments and providing the patient’s family with contact information to follow up on the status of scheduling. Empower families to identify goals and develop patient-specific intervention activities that will achieve realistic outcomes. Explore available resources (both internal and external) to meet proposed goals and problem solve long-term needs. Provide ongoing monitoring of patient/caregiver progress toward identified goals and readiness for discharge from the program. Participate in the collection of utilization data to assess the effectiveness of the Nurse Navigator Program. Participate in internal and external marketing/public relations activities, including dissemination of information in informal and formal education programs. Qualifications
Must have and maintain a current Nursing license from the Maryland Board of Nursing or compact state. Related case management, quality assurance or utilization review certification preferred. EDUCATION: Associates degree or completion of an accredited school of nursing program required. Bachelors of Science in Nursing (BSN) degree preferred. EXPERIENCE: Must have a minimum of three years of experience in a health care setting. Benefits
