Job Description Job Description: Discharge Planning Nurse
Position Summary: We are seeking a dedicated and qualified Discharge Planning Nurse to join our dynamic Case Management team. The ideal candidate will play a critical role in facilitating patient transitions from inpatient care to post-discharge settings. This position operates Monday through Friday from 8:00 AM to 4:30 PM, focusing exclusively on discharge planning for inpatient services.
Key Responsibilities:
- Conduct comprehensive assessments to prepare patients for discharge. - Collaborate with multidisciplinary teams to ensure appropriate discharge plans are in place. - Coordinate with patients, families, and healthcare providers to educate about post-discharge care needs. - Develop individualized discharge plans based on patient assessments and care requirements. - Monitor the discharge process to ensure it aligns with care standards and patient needs. - Maintain thorough documentation in electronic medical records (EMR) using Epic and Care Point systems. - Engage with various patient populations, including cardiac, trauma, oncology, and medical-surgical patients.
Qualifications:
- Registered Nurse (RN) license required. - Basic Life Support (BLS) certification. - Experience in discharge planning is a must. - Familiarity with electronic medical records, preferably Epic and Care Point. - Strong communication and interpersonal skills.
Work Schedule:
- Monday - Friday, 8:00 AM - 4:30 PM - No weekend or holiday requirements.
Dress Code: Navy blue pants and turquoise tops.
Additional Information:
- Past experience with InterQual criteria or Milliman is not required for this role. - This position focuses solely on discharge planning in an inpatient setting.
