The Coordinated Outreach Referral, Engagement (CORE) is a program of Contra Costa Health Services Health, Housing, and Homeless Services division, in partnership with Heluna Health. The CORE program works to engage and stabilize homeless individuals living outside through consistent outreach to facilitate and/or deliver health and basic need services and secure permanent housing.

Salary: $25.20 - $27.76 Hourly

ESSENTIAL FUNCTIONS

Answer 211 phone lines, communicate directly with callers, and provide a high level of customer service to both people experiencing homeless and community stakeholders. Dispatch CORE teams using the ARC GIS dispatch system and keep record of dispatch assignments. Communicate directly with the CORE teams regarding community needs. Provide information about shelter bed availability to CORE teams and alert CCACS, CCYCS, and ECIHP shelters of incoming placements. Provide information about warming center availability and coordinate placements as needed. Run persons seeking shelter and warming center placement through the Megans Law database. Collaborate with team members, including the HMIS Agency Administrator and program management team to support movement or completion of activities and projects. Enter data into the Homeless Management Information System (HMIS) and other various data management systems as assigned. Ensure all CORE HMIS documents are properly stored and secure. Check the Homeless Management Information System (HMIS) for quality assurance purposes and alert CORE teams when intakes and other information are missing from the system. Attend all mandatory meetings and trainings as required. Other duties as assigned. Minimum Qualifications

High school diploma/GED required Preferred: AA/AS or 60 semester college units, or Bachelor's degree.

1 year experience working with computers and/or in an administrative capacity. Preferred experience working with vulnerable or special needs populations. Desired Qualifications:

Bilingual, bicultural in Spanish. Lived experience of homelessness and/or accessing behavioral health services. Previous experience or training in street outreach and clinical case management. Knowledge of Contra Costa County and community resources. Knowledge of the Contra Costa emergency provider network. Prior experience with documentation and billing procedures. Additional Qualifications

Be able to type 40 words per minute. Demonstrate proficiency in Microsoft Office Suite applications and use of the internet. Be fingerprinted and pass a criminal background check.
