NYP Health4Life Program
Part-Time - Queens The Social Worker will play a key role in supporting the Health4Life team and the Senior Director of Maternal-Child Services, working in close collaboration with the program's medical lead. This role involves both clinical social work within the Health4Life clinic and community engagement to enhance access to healthy lifestyles for patients and their families. Key Responsibilities In the Clinic: Conduct psychiatric and psychosocial assessments, therapeutic interventions, psychoeducation, family interventions, and patient advocacy. Serve as the mental health and social services expert on the interdisciplinary team. Support patients and families during their single-appointment visits, working alongside rotating interdisciplinary team members. In the Community: Establish and maintain strong partnerships with community organizations that align with Health4Life's mission. Facilitate weekly parent support groups and assist in organizing community outreach events, occasionally during evenings and weekends. Assist with administrative tasks as needed to ensure the smooth operation of the Health4Life program. This position is ideal for a passionate and dedicated Social Worker who is committed to improving pediatric health outcomes through both direct clinical care and meaningful community partnerships. Preferred Criteria: Experience working with elementary and/or adolescent patients and their families in a community setting Experience working in a community setting Bilingual in English and Spanish Excellent organizational and analytical skills and a high degree of flexibility Required Criteria: Master's Degree in Social Work (MSW) Licensed Masters Social Worker (LMSW) by the State of New York within 1 year of hire Limited Permit (must supply exam date prior to the start of employment) Competency in individual, family and group treatment modalities and treatment techniques appropriate to an outpatient treatment setting Ability to conduct initial evaluations and psychosocial assessments and to establish individualized and measurable treatment goals Ability to communicate effectively and to collaborate within an interdisciplinary team Excellent written and oral communication skills Join a healthcare system where employee engagement is at an all-time high.
